See if the tcp connection is made and keys negotiated correctly.
If so, then it might be that the user isn't allowed to login for another
reason. (I had this problem).

See if the user's name is in the shadow password file. If you vipw to add
a user, the shadow file isn't always updated. (You should add users
through the useradd command, no matter what vipw says about 'all tokens
being updated'.)
