This is a known issue with the Formac cards. I use the NCR53C875 driver,
rather than the Symbios, SYM53C875 you list in your message. If you
append the following to you kernel append string:

ncr53c8xx=safe:y

You will lose some of the advanced features, but it card will work in
Linux. I was never able to get the same functionality with the Symbios
driver.
