init respawning too fast

I know this is "The Most Asked FAQ" but I've looked through hundreds of posts and can't fix this problem.

I've been running YDL 1.2 for months without making any major upgrades/additions. I am now getting the famous :

int: Id "x" respawning too fast: disabled for 5 minutes

where x is between 1-6. I only boot into runlevel 3 and have verified this by /sbin/runlevel

N 3

And I'm not running X.

I don't see any odd messages in the boot.log or the messages file. gdm is not running. I've even tried disabling nfs. The only thing I can
see is in the inittab there are the gettys lines:
1:2345:respawn:/sbin/mingetty tty1
..
6:2345:respawn:/sbin/mingetty tty6

Not sure if this could be related to the problem?
