Make an .Xmodmap file in your home directory:
xmodmap -pke ~/.Xmodmap

Add to the top of the resulting file:
! 3 - 2 Button Swap for Kensington Turbo Mouse
pointer= 1 3 2 4 5

Caveats:
This only works in X. It doesn't change anything in text mode.
The upper right button still doesn't do anything AFAIK.

The .Xmodmap file will "run" automatically every time you log in.
I don't know if it can be made truly global,
but you can put a copy in /etc/skel/, and every user's home directory.
