Which output module are you using? I've had the best luck with the eSound
output module ... the OSS output module gives that apparent
signed/unsigned noise, sometimes. And aRTS just doesn't work (known
problem on PPC).

You configure cddb usage under the XMMS menu, options, preferences.
On the "Audio I/O Plugins" tab, choose the cdread plugin, and hit
configure.

Dunno, I've never used the RPM's for building my kerneles. I'd imagine
that the config is installed when you install the kernel-source for that
kernel version.

As was already mentioned - this is most likely a PMUD problem.

Here's an RPM that incorporates the mentioned patch, no other changes.
This fixes it up for me:
